Thank you for taking the time to give us your feedback, it is much appreciated and really does help us to improve the service we offer to our patients.

Regarding your comments about questionnaires, I think you are referring to your medical history update which we ask you to complete afresh every 6 months in line with current GDC and NICE guidelines; it forms a crucial part of the examination protocol and your ongoing care at the practice.

Often, nurses will complete this with the patient prior to examination, she will then alert the dentist if there are any changes or additions. All patient records, including medical/social history, are treated in the strictest confidence, in line with our confidentiality policy and date protection guidelines. We feel it is imperative that both treating clinician and nurse are aware of potential risks to your safety due to medical conditions. Of course, any direct conversation regarding changes or additions to your well-being will always be between you and the dentist.
